Following on from the needs assessment, admission to the home is initially considered on a trial basis for at least one month.  Continual assessment will be carried out to establish a care plan appropriate to the resident�s criteria of needs. A final decision is made jointly as to the permanency of placement at the home after the initial trial period.

The needs assessment and care plan will determine the level of care each resident will require and forms the basis on which Audit Care�s service is delivered. At the time of admission we will work closely with our resident client and their relative or representative to draw up a written plan of care.  The care plan is dynamic and sets our objectives for care, how we hope to achieve those objectives and any associated risks.  The plan includes the resident�s preferred daily routine, dietary requirements, personal and health care, social and religious interests etc. the plan is reviewed on a regular basis and our clients are encouraged to participate fully in the care plan process.
